First Visit

Your initial appointment will consist of a consultation with Dr. Stanford gathering information, possibly radiographic imaging, and explaining your diagnosis and treatment options. Occasionally, surgery can be performed the same day as the consultation. This is offered in select circumstances. It is understandable that a complex medical history or complex treatment plan may require an initial evaluation and a second appointment to provide treatment on another day.

Please assist us by providing the following information at the time of your consultation:

  • Your surgical referral slip (given to you by your referring dentist, dental specialist, or physician) and any x-rays if applicable.
  • A list of medications you are presently taking.
  • If you have medical or dental insurance, bring the necessary completed forms. This will save time and allow us to help you process any claims.

For an expedited check in process, we encourage you to complete the online health questionnaire via our secure Truform portal on our website prior to your initial visit. 

IMPORTANT: All patients under the age of 18 must be accompanied by a parent or guardian at the consultation visit and at the time of surgery.  

Please alert the office if you have a medical condition that may be of concern prior to surgery (i.e., diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, etc.) or if you are currently taking any medications (i.e., heart medications, aspirin, anticoagulant therapy (blood thinners), etc.).

A preoperative consultation and physical examination is mandatory for patients undergoing IV anesthesia for surgery. 

X-RAYS

If your dentist or physician has previously taken x-rays you may request that they forward them to our office via mail or email. **Please DO NOT FAX over any images, as clarity and quality may be decreased.**  Also the patient may pick them up and bring them to our office. If additional films are indicated/necessary they can be taken at our facility. Cypress Bend Oral and Maxillofacial Surgery also believes that your images are your personal health information. If you would like any of your images sent to other healthcare providers, please complete a health record information release form. If a disc or storage drive is requested, then a small fee will be associated along with any certified mail fees. Privacy and package tracking are very important to us.
